Our Adult Foster Care (AFC) Program supports elderly adults and individuals with disabilities to live safely and comfortably in home settings. We are seeking a dedicated and compassionate Full-Time Care Manager to join our growing team and play a vital role in the coordination, evaluation, and support of care provided to our members.

    About the Role

    Fortcom Health Services is seeking a compassionate and organized Full-Time Care Manager to support our Adult Foster Care (AFC) program. This role is ideal for a licensed social worker who enjoys meaningful work, flexible scheduling, and being part of a mission-driven team. The Care Manager will maintain a caseload of approximately 70–90 members, conducting monthly visits and coordinating care to ensure our members remain safe, healthy, and supported in their homes.

    Key Responsibilities:

    Conduct initial and periodic home visits to assess the AFC setting and member needs.

    Perform social history and ongoing psychosocial evaluations to determine AFC appropriateness.

    Develop, implement, and regularly review individualized AFC care plans in collaboration with the registered nurse and caregiver.

    Train, evaluate, and supervise AFC caregivers in conjunction with the RN.

    Review caregiver daily logs and member documentation at least every 30 days.

    Maintain accurate and timely progress notes for each visit or significant change in condition.

    Coordinate AFC services with other health and community agencies (e.g., VNA, DDS, DMH, MRC).

    Ensure each qualified setting meets safety, health, and regulatory requirements.

    Assist with emergency planning and backup care strategies for each member.

    Provide timely response to urgent or emergent member needs.

    Participate in admissions, discharges, and transitions to ensure continuity of care.

    Make referrals to additional services and supports as needed.

    Qualifications:

    Bachelor’s degree in a relevant field (Social Work, Psychology, Human Services, etc.).

    A current Massachusetts Social Worker License from the Board of Registration.

    Previous clinical experience working with elders or adults with disabilities.

    Strong written and verbal communication skills.

    Reliable transportation and valid driver’s license.

    Excellent organizational, documentation, and time-management skills.

    Proficiency with technology and documentation systems.

    Comfort working independently in the field and managing multiple priorities.

    Job Conditions:

    Ability to conduct home visits throughout assigned regions.

    Must be able to access homes that may not be wheelchair accessible.

    Requires the ability to walk, sit, stand, stoop, and occasionally lift or support members during visits.
