Qualicare Home Care – Greater Austin is seeking an experienced, compassionate, and driven Care Manager to join our team. This pivotal role is ideal for an individual who is passionate about improving lives through thoughtful care coordination, caregiver mentorship, and community engagement.
The Care Manager will conduct all in-home assessments, develop and implement person-centered care plans, and serve as a trusted advisor to clients and their families. The ideal candidate brings strong clinical knowledge, excellent communication skills, and a natural ability to build relationships—with both caregivers and community partners.
Key Responsibilities
Conduct comprehensive in-home assessments for new and existing clients.
Develop customized care plans based on individual needs, goals, and preferences.
Serve as a primary liaison with clients, families, and caregivers to ensure consistent, high-quality service.
Monitor client progress and adapt care plans as needed in collaboration with caregivers and family members.
Mentor and support caregivers to ensure adherence to best practices, promote retention, and encourage professional development.
Lead caregiver mentorship initiatives and promote a team culture focused on compassionate, consistent, and person-centered care.
Design and oversee continuing education programs for caregivers, identifying training needs and facilitating learning opportunities that align with company standards and state requirements.
Participate in interdisciplinary care meetings, staff training, and ongoing quality assurance activities.
Maintain accurate documentation and communication with administrative staff and health professionals.
Build and maintain a strong network of referral partners including social workers, discharge planners, senior communities, and other professionals in the aging and healthcare space to drive new business opportunities.
Participate in community conferences and workshops to promote knowledge in community health trends, efficiency in job performance, and overall professional growth.
Qualifications
RN, LVN, LSW, or similar clinical background preferred, but not required for the right candidate with care coordination experience.
Minimum 4 years of experience in care management, case management, or home care.
Strong understanding of aging-in-place models, chronic care needs, and family dynamics.
Demonstrated ability to lead, coach, and motivate direct care staff.
Familiarity with local senior resources, health systems, and community partners a plus.
Excellent interpersonal and problem-solving skills.
Strong organizational and time management abilities.
