    HomeWell Care Services of NW Indiana wants to consider you for our team! Looking for something where no day is the same that has some flexiblity for your life? Have experience in home care or health care? We are seeking a dedicated, Care Manager to join our team ... one who is experienced, professional and trustworthy, able to take charge from the start, a quick learner, positive-minded, organized, professional, compassionate, and well-spoken. This is a part-time position to begin with that will grow into full time soon. Knowledge of healthcare or in-home care with strong computer skills (Microsoft office) are a must as well as being willing to do some out of the office activities.

    MINIMUM QUALIFICATIONS

    Solid healthcare industry experience preferred, 1-2 years experience in an office environment is helpful.

    Must have a reliable means of transportation with current insurance and a valid, unrestricted driver’s license.

    Must have good computer skills (Microsoft office), attention to detail and grammar, and be able to learn new applications/platforms with ease.

    Must pass a national/expanded criminal background check.

    Must possess impeccable organization skills as well as attention to detail and be able to multitask, exercise initiative, problem solve and apply sound judgment.

    MAIN DUTIES AND RESPONSIBILITIES

    Manage recruiting and interviewing of caregivers.

    Lead and manage caregiver orientation, including updates to program as needed.

    Diligently maintain detailed reports, complete data entry.

    Be responsible for sharing agency after-hours calls as needed.

    Complete assessments, develop and implement detailed written care plans.

    Assign and schedule caregivers to clients.

    Manage call-offs to ensure client coverage.

    Regularly communicate with clients, families, designated client representatives.

    Supervise caregivers.

    Attend and host health fairs and job fairs as needed.

    Perform scheduled and non-scheduled in-home client support visits.

    Complete additional duties and responsibilities as assigned by management and as needed.

    ****NOTE: Position is Part-Time with possibility for Full-Time in the future.
