    Sunday Health is reimaging how care is provided and the structure of the clinical team. We believe in an NP-led model with consulting neurologists where each patient and caregiver dyad is assigned a dedicated care navigator to partner with them throughout their journey. We believe in meeting people where they are and offer remote first care with opportunities for in-person visits.

    About the Care Navigator role

    Sunday Health is seeking a Care Navigator to help accelerate our mission of working with patients and their loved ones. The Care Navigator role is an essential part of the team, helping to coordinate incoming interest and facilitating the entry of the patient into the clinical workflow. This position is an excellent opportunity for someone who is interested in entering the healthcare field and wants to learn and get direct patient experience, or someone who is already in health care and would like to expand their capabilities. The individual in this role will help shape what the care navigator position can look like, working with clinical and product teams on identifying and implementing improvements, preparing training for future navigators, and integrating patient feedback into the process.

    Responsibilities:

    The responsibilities for this position fall into three main areas:

    1. Direct patient interactions

    • Directly work with patients and their loved ones as they enter the Sunday Health process flow
    • Provide introductory information related to the company and our offerings
    • Assist patients and their loved ones throughout the process as their main point of contact

    2. Indirect patient support and advocacy

    • Become a champion of each and every patient who is involved with Sunday Health, working to help in any way they need during the process, including helping them locate resources, collect paperwork for their medical records, and respond to any other inquiries or challenges that arise
    • Search for and provide specific information related to resources for cognitive care such as imaging and infusion centers
    • Act as liaison between the patient and the Sunday Health clinical team

    3. Internal team/patient comms and organization

    • Maintain all aspects of customer relationship communications
    • Work with product and clinical teams to prioritize areas of improvement and work to execute changes
    • Identify challenges to all parts of the process and highlight them for resolution

    4. Other activities related to care navigation

    • Respond to incoming tasks related to this position such as changes in technology tools or clinician workflows by capturing effects and supporting changes

    Qualifications:

    • Education & Licensure:
      • RN or clinical license a plus but not required
    • Qualities:
      • High degree of emotional intelligence.
      • Ability to face challenges with curiosity and creativity.
      • Approach work tasks with flexibility and resourcefulness to find answers/seek support in a timely and professional manner.
      • Driven by customer success and providing an excellent experience.
      • Positive attitude even when faced with difficult situations.
    • Experience (2+ years):
      • Previous patient care is a plus though not required
      • Providing direct customer support
      • Working as part of a team
      • Learning various technology systems and tools.
      • Delivering telehealth services a strong plus.
      • Working within a startup or highly dynamic environment a strong plus.
