    Company Description

Care Partners Medicine was created for you – our patients and our healthcare partners. Our belief is that care should not be about the “episode”, but rather a longitudinal look at each patient’s historical clinical utilization coupled with the patient’s care goals and social and environmental determinants that affect their overall health and patient journey.
