POSITION SUMMARY

The Data Entry Clerk is responsible for the day-to-day administrative support of Care Partners Global Transitional Care. They will commit to learning and demonstrating the Care Partners way: WE WILL CREATE DEEP, MEANINGFUL RELATIONSHIPS WITH OUR CLIENTS, FAMILIES, AND ASSOCIATES THROUGH THE ASPECTS OF LOVE, EMPATHY AND COMPASSION.

• Work schedule: Monday through Friday business hours
• Location: on-site at Orange County corporate office
• Reports to: Vice President of Clinical Services / Chief Financial Officer

YOUR RESPONSIBILITIES

• Work alongside Transitional Care Nurses with frequent communication
• Create spreadsheets to track hospital data
• Verify and transfer patient information from EMR to data tracker
• Collect and synthesize data for the Transitional Medicine team with regular updates
• Support Transitional Medicine team with administrative tasks and special projects as needed

​YOUR QUALIFICATIONS

Required

• Must be at least 18 years old
• High proficiency in Microsoft Office (Word, Excel, Outlook)
• Attention to detail, effective communication, and decision making skills
• Coachability and commitment to problem solving
• Adherence to company policies when handling confidential information
• Must present a professional appearance and promote a positive, conscientious work environment

Preferred

• Data/analytics experience
  • Experience with formulas, pivot tables, etc. if interested in opportunities in analytics
• At least 1 year of experience in an office setting
• A college degree in relevant field
