Care Specialist – Scheduling & Case Management, Silver Care Home Health Services
Position Overview:
The Care Specialist plays a vital role in delivering consistent, high-quality care for Silver Care clients through a case-management-focused scheduling approach. This position goes beyond traditional scheduling and requires a thorough understanding of client needs, caregiver compatibility, policy adherence, and proactive communication. Care Specialists are responsible for managing cases with a client-centered, policy-driven, and solutions-focused mindset, which supports continuity of care, reduces turnover, and enhances client satisfaction.
Core Responsibilities
Case Management & Client Support
Manage assigned client cases using a case management approach.
Understand and apply the 5 W’s for each client:
Who the client is and their care needs
Why the client chose Silver Care
What the client experienced before Silver Care
When consistent contact and follow-up are required
Where care is being delivered and any location-specific considerations
Maintain regular, timely communication with clients and caregivers.
Identify concerns early and take action to prevent service disruptions.
Document all client interactions, changes, and concerns accurately and promptly.
Scheduling & Staffing
Schedule caregivers based on client needs and caregiver compatibility, not just availability.
Fill open cases efficiently while ensuring quality and compliance.
Collaborate with team members to ensure coverage across service areas.
Support the onboarding and integration of new caregivers into cases.
Assist in filling cases across multiple regions as needed.
Policy & Procedure Enforcement
Communicate Silver Care policies and procedures clearly and consistently on all calls.
Base all decisions on policy, facts, and documentation, rather than personal opinions or philosophy.
Educate caregivers and clients on policy expectations to minimize misunderstandings.
Work proactively to reduce unnecessary write-ups, removals, and service failures.
Ensure compliance with documentation standards, including timely placement of notes.
Team Collaboration & Professional Conduct
Work collaboratively with leadership, schedulers, and administrative staff.
Participate actively in training, transition initiatives, and process improvements.
Maintain integrity, honesty, and consistency in all actions and communications.
Demonstrate adaptability during organizational changes and growth.
Support leadership in reinforcing Silver Care’s standards and culture.

Sunday, Monday, Wednesday, Thursday, Saturday: 8 AM - 5 PM
