Summary:

Per diem opportunity for a CNA/HHA to join an established and growing home health care agency. Most work is remote and in the homes of patients. CNAs/HHAs are paid for each completed visit and documentation. The work schedule is dependent on patient needs.

This is an excellent opportunity to work independently providing care to patients in their homes. CNAs/HHAs work collaboratively with nurses and other disciplines providing care, and support to patients and their caregivers.

Responsibilities:

Provide care to patients in their homes as delegated by the care-managing RN.

Observes, records, and coordinates care with multi-disciplines regarding the patient's treatment and any change in the patient's condition.

Supports the patient and family as appropriate to the patient's condition.

Qualifications:

Current certification as a CNA/HHA in the state of Connecticut.

Valid Driver's License

Reliable transportation and willing to travel in New London County and other surrounding towns.

Strong communication and interpersonal skills.

Home health experience preferred.
