    Under the supervision of the Unit Manager, Supervisor or delegate in charge of the team/unit, the Rehab Assistant I / NA assists patients with activities of daily living, provides basic nursing care to non-acutely ill patients, and assists in maintenance of a safe and clean environment.  

    WORK EXPERIENCE:

     

    EXPERIENCE AS A NURSING ASSISTANT PREFERRED. 

    EDUCATION:

     

    High school diploma or equivalent preferred.

     Provides morning and evening care, which may include showers, oral hygiene, combing hair, back care, nail care, dressing patient, straightening room and labeling patient’s personal care items, as needed.

    Provides assistance with ambulation and mobility as well as positioning, lifting, turning, and transferring patients. At JHIRM, employees will follow Safe Handing & Movement of Patients Program

    Makes patient care rounds at least hourly. Reports (and when appropriate, records) observations of patient behavior and uses adequate judgement to notify the RN of any changes in clinical or functional status.

    In conjunction with the nurse (RN/LPN) answers patient call lights promptly and whenever possible anticipates patient needs.

    Provides for exceptional customer service, utilizing interpersonal communication skills and behavior management skills, including positive reinforcement.

    Assists patients with bowel and bladder needs, such as assisting patient to the bathroom, assisting the patient in the use of bedpan, urinal or commode. Monitors and documents intake and output. Applies external catheters. Maintains and cleans urinary drainage bags.

    Obtains and documents vital signs including temperature, pulse, respirations, and blood pressure. Uses good judgement and notifies the RN of any changes or findings beyond normal limits.

    Knowledgeable in FIM documentation and scoring and under the supervision of the RN/LPN is capable of assisting in assessing the patient’s functional abilities and accurate documentation of same as related to the nursing department.

    Assists in monitoring environment for potential safety risks to patient, visitor, and staff. As appropriate, assists RN in reporting, assessment, intervention, and investigating, and follow up on patient safety issues.

    Demonstrates an adherence to and knowledge of infection control policies as evidenced by in-services attendance records and employee practices. (

    Assists in and accompanies patients on the admission/transfer/discharge process and assists in preparing patient for discharge and emergency transfers.

    Prepares patients for meals and assists with feeding patients and with nutritional intake. Must be able to demonstrate the ability to follow dietary restrictions including thickened liquids.

    Assists in preparation of units, patient rooms and beds for

    receiving patient admission and transfers.

    Assists in orientation of new personnel.

    Provides for the proper utilization of patient care supplies and follows the established charge procedure for patient charge items.

    Assists with handling and care of clothing and other personal supplies.

    Assists in the collection of urine and stool specimens

    Transports and accompanies patient to therapies, and outside agency appointments as assigned.

    Attends in-service education programs independently and as assigned, to learn new techniques and procedures, and to maintain current skills.

    Assists with applying and utilizing special equipment such as abdominal binders, anti-embolism stockings, passive range of motion (CPM), donning/doffing orthotics and prosthetics.

  • Qualifications

    MINIMUM QUALIFICATIONS:

    EDUCATION: 

    • Graduated from an accredited school of nursing with Advanced Practice Nurse Master’s Degree OR
    • Graduated from an accredited Professional Physician Assistant Program
    • Active Physician Assistant state license or Nurse Practitioner state license 
    • Active DEA license from state of practice if applicable
    • Board Certified (NP must be FNP certified for six (6) months to adult)
    • BLS Certification
    • Medical Examiners Certification for DOT physicals (ME certification may be acquired within the first 30 days of employment)

    ADDITIONAL KNOWLEDGE, SKILLS AND ABILITIES:

    • Must be aware of hazardous situations and be able to provide a leadership role in emergencies as needed.
