We are looking for a Home Care Aide to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes.

You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs.

Strong verbal and written communication skills are needed to succeed in this role.

RESPONSIBILITIES:

Required services from 12pm to 5pm (Must assist pt. with using a bedpan) Per diem

Required services from 11:45pm to 5:45am (Must prepare both breakfast and lunch) Per Diem

Maintain records of patient care, conditions, progress or problems to report and discuss observations with supervisor

Engage patients in exercises or other activities

Medication reminder, assist with light housekeeping, meal prep. errands & ADL's

Assist with transferring (mobility) and with Bed pan exp. required

Must have Inpatient experience: Preferably a CNA, Medical Assistant or Certified Home Health Aide

Services required: Monday through Sunday Overnights for every other weekend in the Center City area.
