Ceresti Health is seeking a compassionate and motivated Dementia Caregiver Coach to provide remote support, coaching, and education to family caregivers of loved ones living with dementia. This vital role is at the heart of our innovative Caregiver Support Program , a first-of-its-kind digital health model designed to improve health outcomes, reduce avoidable hospitalizations, and enhance the experience of caregiving for families impacted by Alzheimer's disease and related dementias.
The ideal candidate will have experience working with individuals living with dementia, be confident managing a caseload, and excel at building relationships through empathy, encouragement, and education. 2+ years of experience working professionally with older adults or persons living with dementia.
Direct caregiving experience (personal or professional) with persons with cognitive impairment.
Fluency in English.
Must be eligible to work in the U.S.
Ability to work independently in a fully remote setting.
Bachelor's degree in psychology, social work, human services, or a related field.

Excellent communication, problem-solving, and active listening skills.
Comfort navigating technology and learning new platforms (Mac/IOS).

Empathetic, resourceful, and confident managing emotionally complex situations.
Ability to thrive in a fast-paced, change-driven environment.
