Position Summary
The Caregiver provides essential non-medical home care and personal assistance services to clients under the Nebraska DHHS Home and Community-Based Services (HCBS) waiver program. The Caregiver is responsible for ensuring client safety, assisting with activities of daily living, and adhering to all state, federal, and agency regulations while working independently in the client's home.
Essential Duties & Responsibilities
Direct Client Care & Tasks: Implement daily services and tasks as scheduled. Read and understand the Care Plan requirements and specific directions for each client. Work independently to competently complete all authorized tasks, which may include:
Housekeeping: Laundry, cleaning, and cooking.
Personal Care: Bathing, dressing, toileting, personal hygiene, transferring, positioning, ambulating, self-medication assistance, and eating assistance.
Transportation: Transporting the client to and from doctor appointments or shopping.
Companionship: Games and other engaging activities.
Health, Safety & Observation: Look for changes in the client's health condition, such as weakness, confusion, or loss of appetite, and report these changes to your supervisor as soon as possible. Report any safety concerns or problem situations immediately.
Electronic Visit Verification (EVV) & Documentation: Complete proper documentation for services performed and hours worked. Strictly comply with the EVV mandate by checking in at the exact start of the visit and checking out at the end. Failure to use EVV accurately may result in disciplinary action.
Infection Control & Food Safety: Strictly adhere to "Universal Precautions" for controlling infections (including proper handwashing and glove usage) and follow all agency guidelines for Safe Food Handling and Preparation to prevent foodborne illness.
Emergency Response: Respond to emergencies responsibly and without direct supervision. Execute Radiant's Emergency Procedures in the event of client falls, fires, or medical emergencies, including identifying problem situations, calling 911, and notifying the agency immediately.
Mandatory Reporting: Act as a mandatory reporter by law. Immediately report any suspicion of abuse, neglect, or exploitation of a vulnerable adult or child to Adult Protective Services (APS), Child Protective Services (CPS), and/or local law enforcement.
Critical Incident Reporting: Report any critical incidents (injury, death, police involvement) to the agency office immediately so the DHHS Service Coordinator can be notified within the required 24-hour window.
Confidentiality & Boundaries: Maintain the strictly confidential status of all Client and Radiant Home Health Care LLC information. Adhere to the Conflict of Interest policy, do not accept significant gifts, and maintain a work environment free of discrimination and sexual harassment.
Qualifications & Requirements
Age & Identity: Must be eighteen (18) years of age or older and have legal U.S. worker status.
Physical Requirements: Must be able to lift twenty-five (25) pounds and be physically fit (able to reach above the shoulders, bend forward from the waist, and bend at the knees).
Transportation: Have the ability to travel to the Client’s residence using personal transportation with the appropriate insurance, or reliable public transportation.
Clearances (Strict Compliance): Must have an acceptable Criminal Background Check (CBC) prior to hire and submit to ongoing mandatory screenings, including:
Monthly: OIG LEIE, SAM.gov, and Nebraska Medicaid Excluded Providers List.
Annually: Nebraska State Patrol Criminal Check, APS/CPS Central Registries, and Sex Offender Registry.
Training & Skills: Successfully complete all Radiant Home Health Care LLC required training within Ninety (90) days of the hire date. Must possess knowledge of basic first aid and know how to obtain medical assistance in a client emergency.
Health Status: Must be free of communicable diseases, physically capable of performing required work, and willing to provide a physician's verification of health status.
Drug-Free Workplace: Must comply with the agency's strict drug-free workplace policy.
