DESCRIPTION
Community CARES Homeless Services (CARES) is a non-profit organization providing homeless services for individuals and families in Cumberland County. Our mission is to serve and strengthen the community by providing emergency shelter, resources, and supportive services to individuals and families at risk of or experiencing homelessness.
Community CARES’ Family Shelter Case Manager will be a motivated individual who is invested in our mission of providing more than a shelter. A person who is driven by and dedicated to the work they do and inspires others to be as well. Principal Responsibilities:
· Manage a caseload of requiring utilizing a Housing First approach.
· Manage a caseload of families while providing stabilization services as outlined in the Stabilization Services policy.
· Engage in rapport building with participants and provide service from a “choice” perspective while utilizing participant-centered, harm reduction, and strengths-based approaches.
· Provide case management services including assessment, budgeting, employment search, arranging childcare, and creation and monitoring of housing plan. Refer/connect participants with appropriate services such as financial assistance, entitlements, addiction services, advocacy, employment assistance, financial counseling, rental assistance, deposits, Domestic Violence services, Mental Health services, etc.
· Document case management services through detailed and thorough case notes in ETO for each family to include: weekly meetings, client and collateral contacts each week, as well as any significant events and/or interactions.
· Enter data into HMIS/Charity Tracker that complies with data integrity and data quality standards.
· The Family Shelter Case Manager should be a well-organized and energetic team player with good judgment, the ability to work independently, and effectively engage a diverse range of clients and community stakeholders. The successful candidate will be self-directed and “seasoned”, with an excellent understanding of Eastern CoC Written Standards, and the abilities and skills to move smoothly between responsibilities and assignments.
· Connect with landlords and realtors to familiarize them with the Community CARES program and create a network of housing opportunities.
· Accompany clients in housing search and outreach to available housing options. Apply knowledge of residential lease contracts and landlord laws to educate clients about their rights and responsibilities.
QUALIFICATIONS:
*Experience working with people from very diverse backgrounds
*Excellent communication and conflict-resolution skills
*Ability to implement and utilize limit-setting techniques and appropriate boundaries
*Ability to pass a background check
*BA (required), will consider: Associate's Degree or equivalent (minimum of 34 credit hours) in Mental Health, Psychology/Social Services, or Substance Abuse or relevant experience will be considered in lieu of a degree,
*Three - five years’ experience
This is a 40-hour-a-week position with benefits including health insurance, paid holidays, and earned time off. The hourly rate is dependent upon experience.
