Summary Statement:
The role of the Family Care Specialist is to support the LYNC patients and children with any and all non-related treatment necessities and matters to ensure the highest quality of patient care is sustained, and that needs are met on a daily basis. This role will support the mission, vision, values and goals of Casa Esperanza as an organization, and ensure smooth and effective functioning of the LYNC residential treatment program.

Principal Responsibilities :
- Provide individual counseling, risk assessments, substance abuse and co-occurring conditions from a needs management perspective
- Conduct outreach and case management referrals to related parenting activities
- Conduct initial assessment for each Client and generate individualized treatment and service plans as appropriate.
- Locate previous residential clients in order to engage them in Outpatient services, through outreach to streets, shelters, educational sessions, etc.
- Manage the follow-up component for a relapse prevention evaluation conducted in collaboration with Boston University by outreaching to participants from across Massachusetts to complete the questionnaires at 6 and 12 months.
- Complete Baseline Questionnaires as needed
- Refer participants and others to appropriate treatment and services
- Complete intake paperwork as necessary for these clients to join services specifically to program or grant enrollments.
- Participate in program development with outpatient clinical team regarding family engagement.
- Work closely with clinical team to ensure program goals are met.Facilitate a peer led support group of relapse prevention.
- Work collaboratively with Boston University staff to complete evaluation.
- Through the outreach process, educate others on the treatment services available at Casa Esperanza, Inc. (residential, outpatient, supportive housing)
- Collaborate with other agencies, public organizations, and external personnel to develop a comprehensive network of servicesMaintain all client records and necessary reports
- Participate in weekly clinical meetingsCoordinate admissions to the program and admissions intake
- Bi-weekly supervision with Program DirectorAssist in all reporting as required by Program Director
- Other duties as required

Education/Training Required
- Minimum Bachelor’s Degree preferred
- Certified Alcohol and Drug Counselor/Certified Alcohol Counselor preferred
- Bilingual Spanish/English required
- CPR Certified preferred
