AGENCY SUMMARY: Casa Esperanza, Inc. is a bilingual/bicultural behavioral health treatment provider serving Boston and the Merrimack Valley. Our mission is to empower individuals and families to recover from addiction, trauma, mental illness, and other chronic medical conditions; overcome homelessness; and achieve health and wellness through comprehensive, integrated care.
JOB SUMMARY STATEMENT: Casa Esperanza is currently seeking a motivated and detail-oriented individual to collect Government Performance and Results Act (GPRA) data for the various federal grant projects that Casa Esperanza manages. Casa uses this data to determine gaps in services for the Latino community in Greater Boston and determine how to best advocate for increased access and quality of care for our population.
The Grant Data Collector will work with the Multidisciplinary Team at Casa Esperanza to determine eligibility for the various grants. After confirming eligibility, Grant Data Collector will conduct initial interviews with patients and work in collaboration with the direct care staff to engage patient in services offered. The Grant Data Collector will also conduct comprehensive outreach to patient to complete follow ups interviews as needed for each grant the patient was enrolled in at the time of their initial intake. Furthermore, Grant Data Collector will make every effort to reengage patients in other services such as individual and group therapy, medical necessity, and other services within the Recovery Support.
Grant Data Collector will also work in collaboration with Casa’s various evaluation teams to ensure we are meeting service expectation for each grant. Grant Data Collector reports to the Lead Grant Data Collector. They will provide support with reports, complete and submit administrative paperwork, and comply with data security and confidentiality requirements. Protecting the health and safety of our employees and survey participants is a top priority for Casa Esperanza Inc. Based on recommendations from the CDC and other public health authorities, we require staff to protect and monitor their health while working on their assignment, which may include the use of personal protective equipment, regular testing for COVID-19, or other screening activities.
RESPONSIBILITIES:
Grant Data Collector
- Describe all the Grants, eligibility criteria and services provide under each grant and follow up with referring eligible patient to the to recovery support services after intake is completed
- Complete the eligibility SAMSHA screening tool, Locator sheet and grants consents form with each patient at intake interview and complete the required documentation in our Electronic Health Record Support patients in completing evaluation questionnaire, including reading questions aloud for individuals with limited literacy, and clarifying questions
- Request, track and distribute incentive to patients after completing follow up interviews when applicable
- Conduct regular and intensive outreach within the community and work with the Outreach Coordinator to reach patients that have disengaged from services at FUOP
- Provide ongoing education regarding the substance abuse, mental health, HIV, and other health and wellness information to clients and their family members
- Work with project staff to help clients feel connected to the recovery community, especially in recreational activities in a drug free environment.
- Follow all Safety and confidentiality guidelines stablished by 42CFR and HIPPA
- Ensuring patients are connected/reconnect to the Recovery Support Services team
Administrative
- Document all client encounters and other data as required and maintain up-to-date client records
- Ensure data collection requirements are completed within specified time frames
- Attend bi-weekly supervision
- Conduct administrative grant discharge based on the grant disengagement criteria for disengaged patients
- Write and submit all reporting as required by Program funding sources in a timely manner
- Attend all relevant case conferences and staff meetings including Multidisciplinary Team Meeting, Office Based Opioid Treatment and meeting with the evaluator team as needed
- Represent the agency by providing education and participating in community events
- Ensure program is meeting state and federal standards of care and reporting requirements
- Support with all bi-annual, annual, and final reports and ensure they are completed and submitted within specified time frames
- Maintain on going contact with project evaluators
- Other duties as assigned
QUALIFICATIONS:
- Bilingual Spanish/English required.
- Knowledge of Substance Use Disorder and Mental Health resources
- Experience doing outreach to individuals at risk for HIV, addiction, mental illness, trauma, and violence.
- Able to work weekends and evenings, as needed
- Familiar with social, health and cultural issues specific to the Latino population
- Ability to work as a team member & independently
- Understanding of harm reduction techniques
- Strong verbal and written communication skills
- Strong interpersonal skills and crisis management skills
- Knowledge of HIV/AIDS health screenings and service planning and education
- Minimum High School Diploma required.
- Computer skills, minimum Microsoft Word
- Valid Massachusetts Driver’s License
