We are seeking a compassionate and dedicated Case Manager/QMHP to join our team. The ideal candidate will have a strong background in case management and a passion for helping individuals in the behavioral health field.
Qualified candidates MUST have a minimum of a Bachelor's a degree in: Psychology, Social Work, Sociology, Psychology, or Counseling.
Duties/ Responsibilities • Conduct thorough assessments of clients’ needs and circumstances. • Develop and implement individualized case management plans. • Provide behavioral support to clients. • Collect data to monitor client progress and outcomes. • Coordinate care with other professionals, including healthcare providers and educators. • Advocate for clients’ needs within the community and healthcare systems. • Maintain accurate documentation of client interactions and case progress.
Minimum Skills /Requirements
Bachelors Degree or higher in psychology, social work, sociology, educational psychology, or counseling
Good listening, verbal, and written communication skills
Good work ethic to complete tasks independently
Functional knowledge of computer programs (Word, Excel, Web based programs)
Must meet state requirements for QMHP status (CANS and ANSA Cert)
Experience working with youth or families in a behavioral health setting
Good driving record with at least 3 years of driving history
Reliable vehicle with insurance
Bilingual in English and Spanish preferred but not required
