    Job Title: Case Manager - Texas HCS Program Reports to: Program Director Employment Type: Full-time

    Job Summary: The Case Manager for the Texas Home and Community-based Services (HCS) program will be responsible for providing comprehensive case management services to individuals with intellectual and developmental disabilities. This role involves coordinating care, facilitating access to services, and ensuring compliance with state regulations to promote the overall well-being and independence of clients. Key Responsibilities:

    Client Assessment:

    Conduct initial assessments and ongoing evaluations of clients to identify needs, preferences, and goals.

    Develop individualized service plans (ISPs) in collaboration with clients, families, and interdisciplinary teams.

    Service Coordination:

    Coordinate and manage services for clients, including medical, therapeutic, and community resources.

    Facilitate communication among clients, families, service providers, and other stakeholders.

    Monitoring and Advocacy:

    Monitor the effectiveness of services and support, making adjustments as needed.

    Advocate for clients' rights and access to appropriate services, ensuring compliance with the Texas HCS program standards.

    Home Checks:

    Conduct quarterly home checks to ensure compliance with home management practices and quality of care standards.

    Documentation and Compliance:

    Maintain accurate and timely documentation of all client interactions, assessments, and service plans in accordance with state and organizational requirements.

    Ensure compliance with all federal and state regulations related to case management and service delivery.

    Education and Support: Provide education and support to clients and families regarding available resources, service options, and community programs.

    Assist clients in developing self-advocacy skills and strategies for achieving their personal goals.

    Collaboration:

    Work closely with a multidisciplinary team, including physicians, therapists, and community organizations, to ensure holistic support for clients.

    Participate in regular team meetings, training, and professional development activities. Key Skills to possess

    Ability to work independently and as part of a team.

    Proficiency in case management software and Microsoft Office Suite.

    Knowledge of Texas HCS program regulations and community resources.

    Working Conditions:

    Primarily office-based, with occasional travel to individual homes and community locations.

    May require evening or weekend hours to accommodate client needs.

    Qualifications:

    Education: A bachelor’s degree in social work, Nursing, Public Health, or related field. A master’s degree in social work (MSW) or related field is preferred.

    Experience: Minimum of 3-5 years of experience in case management, healthcare, or social services, with at least 1-2 years of experience in leadership or supervisory role.

    Knowledge: Understanding of home and community-based service delivery models, healthcare systems, and regulatory requirements. Familiarity with Medicaid and Medicare programs is a plus. Proficient MS Office Suite
