    Position Title: Case Manager Reports to: Director of Housing and Operations

    Employment Status: Full-time, 32 - 40 hours FLSA Status: Non-Exempt, hourly

    Hours: Monday - Friday 8am - 5pm. Nights and weekends as needed.

    Job Summary

    The Case Manager provides case management services and support to participants in the sober living housing program at Fresh Coast Alliance.

    Responsibilities:

    Participate in the screening and selection of applicants who are seeking recovery/re-entry based sober living.

    Provide orientation to incoming housing participants to outline program rules and expectations.

    Conduct assessments with participants to determine goals, barriers, resource needs and other factors related to case planning, referring them to community resources as appropriate.

    Meet with participants on a weekly basis to review and discuss progress in the program.

    Maintain accurate and updated records of all interactions with clients and any other contacts made on their behalf. Document participants progress toward program goals.

    Provide on-going monitoring and reporting of participant’s progress. Communicate clients’ progress by conducting weekly interdisciplinary meetings and evaluations;

    Attends all meetings and trainings as required

    Verify clients’ attendance at any groups, treatment sessions or any other services that are required as part of the participants program plan.

    Enforce rules of the program and provide disciplinary action for any rule violations.

    Intervene in crisis situations.

    Coordinate with the treatment team, recovery coach, Director of Housing and any other necessary individuals who are involved in the program planning for each participant.

    Work with participants to develop an exit plan and establish goals consistent with this plan.

    Prepare clients’ discharge plans; coordinating discharge and post-discharge requirements; providing resources.

    Other duties as assigned.

    Minimum Qualifications:

    An Associate's degree in Human Services, Social Work, Criminal Justice, or a related field is required; a Bachelor's degree is preferred.

    Demonstrated experience with long-term sobriety or prior incarceration is strongly desired; a minimum of two years of sustained sobriety is mandatory.

    A minimum of two years of case management experience within a clinical/social service environment or direct experience working with clients possessing a history of substance use and/or incarceration is preferred.

    Peer Recovery Coach certification is advantageous.

    Comprehensive knowledge of community resources and service programs for individuals with a history of substance use and/or incarceration is required.

    A thorough understanding of the substance use recovery process and the distinct needs of formerly incarcerated individuals is essential.

    A valid driver's license is a prerequisite.
