Summary of Duties: The Case Manager is responsible for the delivery of comprehensive case management services to assigned clients from the Compass House Shelter and Resource Center components. Utilizing solution oriented strength-based counseling techniques, case managers role model, instruct and reinforce the acquisition of independent living skills by clients, regarding the clients as full and equal partners in the counseling process. Case managers model appropriate empathy and nonjudgmental acceptance, employing relationship-building techniques that enhance and nurture the development of a functional helping relationship.
Accountability: The Case Manager is accountable to the Director of Youth Services, who is responsible for ongoing training, supervision, and mentoring.
Responsibilities and Examples of Work:
Conduct formal counseling sessions with each assigned Shelter resident on a daily basis and with Resource Center clients at least once per week.
Conduct and/or complete comprehensive assessments of each client’s functioning in designated life areas as initiated in the Intake process.
Develop with client an individualized service plan utilizing solution oriented strength based principles to enhance life skill areas assessed as deficient and to develop case direction. This plan will be maintained and updated in the client’s file throughout the client’s involvement with Compass House.
Model, prompt, role play and instruct clients in life skills necessary to maximize the development of a stable living arrangement, based on the client’s particular case direction.
Work with Shelter staff to assist client develop a functional adjustment to group living, utilizing solution oriented strength based counseling techniques and modeling appropriate adult communication and problem solving behaviors.
Establish and maintain joint case planning efforts with all agencies significantly involved in the delivery of services to the client.
Develop a thorough and current knowledge of all community service resources available to the Compass House client population.
Link youth to appropriate community services and monitor progress as appropriate within the case plan.
Perform case specific advocacy for clients whose entitlements have been jeopardized by barriers to effective service delivery.
Accompany client wherever possible to role model assertive communication techniques to maximize the opportunity for effective service delivery.
Coordinate service delivery from appropriate community resources, devoting particular attention to re-establishment of connections within the natural support systems in the client’s extended family, neighbors, friends, ethnic, cultural, and religious communities.
Initiate efforts to involve the youth’s primary care givers in assessment and case planning prior to the establishment of case direction.
Make periodic home visits to Resource Center clients at a minimum of eight visits per month per case manager.
Assist in training, mentoring, supervision and evaluation of student interns and volunteers.
Maintain a safe, clean, orderly environment for residents and other staff.
Maintain records such as logs, contact sheets, and other pertinent information in a timely and dependable manner.
Share pertinent information about residents and program with other staff, volunteers, and interns.
Participate in staff meetings, in-service training, and supervisory sessions as scheduled.
Any other task or responsibility or responsibility as requested by Supervisor or Director.
Special Conditions: Case Managers generally work Monday through Friday, 9:00 a.m. to 5:00 p.m. schedule, but must be available to be reached as needed for emergencies. Because of the extensive field-
work necessary, the Case Manager must have regular use of an automobile.
Qualifications: Bachelor’s degree in human services related field and two years paid work experience with youth in recreation, childcare, counseling, teaching, or related equivalent as determined by the Compass House Administrative Team.
