    Overview

    The Case Manager is a field-based professional responsible for providing support to the unhoused and vulnerable population and connecting them to healthcare and social services. The Case Management team builds trust with potential members through direct engagement and partnership. This role requires a blend of administrative precision, initiative, and compassionate, one-on-one advocacy.

    Key Responsibilities

    Care Coordination & Advocacy

    Manage a dedicated caseload of members, developing individualized service plans to improve goal outcomes.

    Act as a primary liaison between members, healthcare, and social services.

    Monitor progress toward goals, adjusting service plans as needs evolve regarding housing, nutrition, and mental health.

    Direct Member Support

    Conduct field visits and assigned meetings to assess the member’s environment and provide hands-on support.

    Assist members in navigating complex systems, including Social Security, Medi-Cal, and CalAIM Community Supports.

    Facilitate "warm handoffs" to specialized providers for behavioral health, medical care, or emergency financial assistance.

    Documentation and Compliance

    Maintain detailed, accurate, and timely case notes in data management systems.

    Ensure all activities meet privacy (HIPAA) standards.

    Engage and document each enrolled member at least two times a month.

    Educate and create referrals to potential new members and enter into the data system.

    Requirements

    Knowledge: Strong familiarity with the Medi-Cal, CalAIM, and ECM services.

    Skills: Exceptional organizational skills and the ability to manage multiple high-priority cases simultaneously.

    Transportation: Valid driver’s license and reliable transportation for frequent travel within Merced County.

    Communication: Ability to advocate effectively in professional settings (e.g., social agencies and board meetings) and communicate empathetically with members.

    Qualifications

    Preferred minimum of 2 years in case management, social work, education, or human services.

    Demonstrated success in case planning and goal attainment for vulnerable populations.

    Bilingual ability (Spanish/English) is highly preferred to serve the Merced County community.

    Proficiency with mobile technology and case management software.

    Success Measures

    High rate of goal attainment within individualized service plans.

    Growth of the referral partner network, the number of successful member enrollments, consistent documentation of contact efforts, and accuracy of documentation.

    Compliance with documentation deadlines and data accuracy.
