Case Manager
The Case Manager is responsible for reviewing child records to ensure they are up to date. Schedule all appointments, attend school intake and the supervision of male residents 6 to 17 years of age in a treatment setting. The Case Manager must ensure that all residents are safe and secure within the scope of each youth’s individual service plan (ISP). The Case Manager may; record daily notes of the youth behavior, update service plan, as well as monitor phone calls and visits of youth in care.
ESSENTIAL FUNCTIONS:
-Act as the central point of contact for all services related to each youth, including education, therapy, healthcare, probation, and child welfare.
Provide regular updates regarding youth progress, concerns, and upcoming appointments or court dates.
Maintain accurate, timely, and confidential case records including assessments, progress notes, service plans, incident reports, and discharge summaries.
Begin discharge planning early in placement to ensure smooth transitions to home, foster care, group homes, or independent living programs.
Maintain ethical boundaries, cultural competence, and a trauma-informed approach in all interactions.
QUALIFICATIONS:
Must have a Bachelors Degree
Bachelor’s degree in the behavioral sciences, human services or social services fields.
Child welfare and/or case management experience is strongly encouraged.
A minimum of two year of related work experience, but 3-5 years preferred.
Must be detail oriented.
Must be a self starter and finisher.
QUALIFICATIONS:
Must be 21 years of age or older.
Flexible schedule (evening, weekends, holidays) to facilitate program services.
Must have a valid driver’s license and be eligible to drive to facilitate program services as required.
Knowledge with grant writing highly preferred.
