For nearly 40 years, Stirling Behavioral Health Institute has been providing high quality mental health services to children, adolescents and young adults ages 0-24 and their families through a contract with the Los Angeles County Department of Mental Health.

POSITION SUMMARY

The Case Manager is responsible for providing office/telehealth case management services, linkage, resources, and rehabilitation services to clients and their caregivers currently receiving services. The case manager also provides parenting skill workshops in our office, virtually, and in the schools we serve.

ESSENTIAL DUTIES & RESPONSIBILITIES

Reporting to the Clinical Supervisor, the Case Manager is responsible for the following:

Maintain all appropriate records.

Complete all required documentation in accordance with LA County Department of Mental Health (DMH) and Agency regulations.

Interview clients individually or in families assessing their situations, capabilities, and problems to determine what services are required to meet their needs.

Serve as liaisons between students, homes, schools, family services, doctors, and other contacts to help children who face problems, such as disabilities, abuse, or poverty.

Administer treatment outcome measures (CANS-IP, PSC-35).

Counsel or aid family members to assist them in understanding, dealing with, or supporting the client.

Refer client or family to community resources, following through to ensure service efficacy.

Educate clients or community members about mental or physical illness, abuse, medication, or available community resources.

Assist clients in adhering to treatment plans, such as setting up appointments, arranging for transportation to appointments, or providing support.

Refer clients to community resources for services, such as job placement, debt counseling, legal aid, housing, medical treatment, or financial assistance, and provide concrete information, such as where to go and how to apply.

Provide, find, or arrange for support services, such as childcare, homemaker service, prenatal care, substance abuse treatment, job training, counseling, or parenting classes to prevent more serious problems from developing.

Provide parenting skill workshops in the office, virtually and in the schools we serve.

Performs other duties as assigned.

Attend appropriate DMH-related meetings for case managers as assigned by supervisor

BASIC QUALIFICATIONS

Bachelor degree and four (4) years documented experience in a mental health setting or an Associates degree and six (6) years of experience in a mental health setting.

Bilingual Spanish/English REQUIRED

Ability to work collaboratively with all co-workers

Proactive, independent individual with strong problem solving skills

Excellent written and verbal communication skills;

Well organized, detail oriented and skilled with time/task management, able to adapt to changes

Ability to work effectively within a multicultural team and agency

Working knowledge of community resources that address the special needs of the client base

Ability to model positive behaviors and agency values

Strong computer skills, Microsoft Outlook, Excel, Word and web-based Electronic Health Record software
