TITLE: SSVF Case Manager
SUMMARY: This position requires knowledge of case management and working with the homeless population.
SUPERVISOR: Project Coordinator
QUALIFICATIONS:
Preferred Bachelor’s degree in Human Services and 1 year of experience providing case management to the homeless population.
Minimum Associate’s degree or working on a Bachelor’s degree in Human Services with 2 years of experience working with the homeless population.
Experience working with various types of ethnic groups.
Pass drug screening.
Reliable transportation.
Familiarity with community resources.
Provide three letters of reference.
Maintain regular attendance.
Able to perform work activities in a high-pressure and time-sensitive environment.
RESPONSIBILITIES:
Assists clients in identifying areas of need and prioritizing meeting the needs.
Develops an individual case management plan.
Coordinates services delivering thru in-house or community referrals.
May secure supplementary information such as medical records, etc.
Assists with transportation.
Serve as liaison with community resources.
Coordinates with local coalitions in fulfilling project goals.
Prepares reports on a timely basis.
Monitors progress and oversees client case files.
Establishes working/referral networks.
Promotes a positive image of NCHP.
Conduct office, home, and field visits to Veterans.
Other duties as required.
