    Guidance with benefits enrollment (health insurance, income support)

    On-site counseling, therapy, support groups, and assistance with emotional well-being and relationship challenges

    Life skills development to foster independence

    Community reintegration through social activities and connections

    Transportation and accompaniment services

    Additional support to achieve long-term housing stability

    Join Our Team:

    JOB SUMMARY

    We are seeking a compassionate and dedicated Case Manager to support justice-impacted individuals facing homelessness or housing instability. They will provide case management, coordinate resources, and address medical, mental health, and substance use barriers, ensuring participants receive support for long-term stability.

    This position is fast-paced and participant-facing; the successful candidate must have a lot of patience, and be comfortable working directly with people experiencing homelessness, mental illness, substance abuse, and domestic violence.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    Conduct intake assessments to determine participant needs, including housing, mental health, substance use, medical care, and social services.

    Develop and maintain Individual Service Plans (ISPs), updating them regularly based on participant progress and needs.

    Connect participants to community-based services, including behavioral health, substance use treatment, housing, food assistance, and transportation.

    Accompany participants to medical, housing, and social service appointments as needed.

    Advocate for participant needs with healthcare providers, ensuring equitable access to services.

    Utilize harm reduction principles to support participants where they are in their journey, reducing barriers to care and engagement.

    Provide health education, self-management coaching, and overdose prevention training to participants.

    Engage in motivational interviewing and trauma-informed approaches to build trust and promote participant-driven solutions.

    Encourage treatment adherence and assist in medication management support.

    Accurately document case notes, assessments, referrals, and care plans in the funders database.

    Track participant progress and collect data for reporting to funders and program evaluation.

    Participate in weekly case conferences with clinical teams to discuss participant needs and challenges.

    Maintain strict confidentiality and compliance with HIPAA and agency policies.

    Liaise with other providers within the agency, as well as outside providers to formulate strength-based strategies that focus on housing retention and general wellness.

    Participate in staff meetings, individual and group supervision, case conferences and training as required.

    Other duties as assigned.

    SKILLS AND QUALIFICATIONS:

    Minimum of 1 year experience working with people who experience homelessness and or are diagnosed with a mental-illness or Social Work/Mental Health related bachelor's degree.

    Experience working with people who are involved with the judicial system and/or homeless is preferred.

    Knowledge of harm reduction, trauma-informed care, and motivational interviewing techniques.

    Have a California driver's license and insurance with a good driving record during employment.

    Recent Physical and TB Report within past year

    Stand or walk on various surfaces for long periods of time

    Previously worked at a traditional housing facility

    License/Certification:

    BLS/CPR Certification
