Case Managers/FSM partner with program participants and families to build mutually trusting relationships, serve as an advocate for families, and provide support services as needed. The Case Managers/FSM support families and program participants in prioritizing their needs and developing goals to achieve long-term success. Additionally, the Case Manager/FSM will identify, develop, and maintain community partnerships and resources that promote health equity and mental wellness.
Scope of Responsibilities:
In collaboration with Outreach teams, CMs/FSS recruit participants and family caseloads.
Program Participant caseload is based upon Outreach team’s caseload.
However, each case manager should have a minimum of five families to support.
Provide case management services to families and program participants, including direct services to families by assisting them in determining family strengths, needs, and goals.
Conduct initial and ongoing assessments of family needs; develop resources; provide referrals and supportive services to assist families in meeting identified needs; and document and report on all social service activity, as required.
Identify and develop resources to meet identified health, social service, and mental health needs; develop effective working relationships with related and relevant resources.
Participate in regularly scheduled team meetings to plan for and deliver collaborative services.
Meet regularly with program participants, families, stakeholders, and other staff regarding the progress of family and program participants.
Conduct home visits for purposes of assessment and support.
Discreetly handle sensitive information and maintain confidentiality at all times
