    Job Description

    We are seeking an experienced and compassionate RN Case Manager to join our dynamic clinical care coordination team. This role involves guiding patients through their care journey, ensuring appropriate resource utilization, timely discharges, and coordination of interdisciplinary care plans.

    Responsibilities:

    • Conduct comprehensive medical and psychosocial assessments

    • Develop and implement individualized case management plans

    • Lead interdisciplinary rounds to ensure alignment on patient care goals

    • Evaluate medical necessity and level of care, utilizing approved criteria

    • Perform utilization review and liaise with payors

    • Facilitate discharge planning and patient/family education

    • Identify and resolve barriers to patient throughput

    • Document interventions and communicate effectively with all stakeholders

    • Track variances and implement strategies for quality improvement

  • Qualifications

    Qualifications

    • Minimum 3 years of recent RN Case Management experience in an acute care hospital required

      • OR at least 3 years combined acute care experience, including at least 1 year within the last 5 years (for home health or insurance case managers)
    • Acceptable background includes: Med/Surg, Tele, Neuro, ICU, PCU, or ED

    • Associate Degree in Nursing or Nursing Diploma required; BSN preferred

    • Florida RN license or compact RN license (must obtain FL license within 90 days)

    • Certification in Case Management, Nursing, or Utilization Review preferred
