    Position Summary

    The Registered Nurse (RN) provides skilled nursing care to clients in their homes in accordance with the physician’s plan of care, agency policies, and state regulations. The RN ensures the delivery of safe, compassionate, and competent care that supports each client’s health, independence, and dignity reflecting Blessing Hands Home Care LLC’s values of faith, integrity, and service to others.

    Key Responsibilities

    Perform comprehensive nursing assessments upon admission, during visits, and at discharge.

    Develop and implement individualized Plans of Care (POC) under the direction of the physician.

    Provide skilled nursing services, including wound care, IV therapy, medication administration, and disease management education.

    Communicate effectively with physicians, clients, families, and the interdisciplinary team.

    Document all care and communication accurately in accordance with standards.

    Monitor client responses to care and update the POC as needed.

    Educate clients and family members on health maintenance, safety, and home care procedures.

    Ensure compliance with Home and Community-Based Services (HCBS) and CMS regulations.

    Participate in case conferences, quality assurance, and continuing education as required.

    Report any incidents, changes in condition, or potential abuse/neglect immediately per agency policy.

    Qualifications

    Current RN license in the state of Missouri (multi-state compact license preferred).

    Minimum one (1) year of clinical nursing experience, preferably in home health, hospice, or acute care.

    Knowledge of OASIS documentation and Medicare/Medicaid home health regulations preferred.

    Current CPR certification (AHA or Red Cross).

    Reliable transportation with valid driver’s license and auto insurance.

    Strong communication, critical thinking, and time-management skills.

    Work Schedule

    PRN: Flexible scheduling based on agency needs and nurse availability.

    Part-Time: Regularly scheduled hours including some weekends or evenings if required.

    Physical Requirements

    Ability to travel to client homes.

    Ability to lift up to 50 lbs and perform prolonged standing, bending, and reaching.

    Must be physically able to perform job functions and pass a Health Assessment per agency policy.
