About the Role:
S.M.I.L.E. INC in Milwaukee, WI is looking for a compassionate and driven Case Manager to join our dedicated team. Responsibilities:
Assess client needs and develop individualized service plans to support their goals
Coordinate and connect clients with community resources, social services, and support programs
Monitor client progress and adjust care plans as needed through regular follow-ups
Maintain accurate and up-to-date case documentation and client records
Collaborate with internal team members, external agencies, and service providers
Advocate on behalf of clients to ensure access to appropriate services and support
Conduct intake assessments and crisis intervention when necessary
Requirements:
Bachelor's degree in Social Work, Human Services, Psychology, or a related field
Prior experience in case management, social services, or a related role preferred
Strong knowledge of community resources and social service systems in the Milwaukee area
Excellent communication, organizational, and problem-solving skills
Ability to manage a caseload effectively while maintaining attention to detail
Empathetic, culturally competent, and client-focused approach to care
Proficiency in case management software and Microsoft Office Suite
