    Position Description:

    Reporting to and in partnership with the Program Director, the Case manager will spearhead referral and support programs for the guests of the Samaritan Center. This position is responsible for working directly with Samaritan guests to provide assessment of individual needs and linkage with concrete community resources or development of appropriate programming to address identified needs. The case manager must be able to combine skillful questioning and attentive listening with support and guidance in a process that gives individuals a sense of control and empowerment over their situation.

    Responsibilities:

    o Work with guests to develop a case plan to meet their needs by providing information, referring for service, coordinating with service providers and/or encouraging natural supports and networks.

    o Develop and implement appropriate programming to meet guest needs as identified through semi-annual guest needs surveys; engaging guests, volunteers, service providers and the larger community as needed and appropriate.

    o Maintain records and files on programming, referrals and cases managed.

    o Provide program and referral service information and data in support of funding requests.

    o Maintain active, visible presence for guests of the Samaritan Center, including scheduled hours outside of meal service hours.

    o Cultivate and maintain strong community relationships with area service providers and organizations.

    o Expand referral knowledge and network through site visits, participation in community task forces and networking.

    o Support the public relations and communications function of the organization through public presentations and generation of relevant internal data.

    o Support the continued population and expanded use of donor database functionality to support the needs of the organization.

    o Support the general operating of the program. As a small team, we all chip in when needed, meaning everything from administrative tasks to picking up donations.

    o Perform other duties as assigned.

    o Strong problem solving skills and the ability to think creatively and conceptually.

    o Ability to handle multiple tasks, with attention to details and deadlines.

    o Working knowledge of Microsoft Office Suite products (Word, Excel, Power Point, etc.).

    o Ability to work independently, under supervision and in a team-based environment.

    o Adaptable and flexible to shifting opportunities, priorities and needs.

    o Ability to communicate and work well with a range of constituents: volunteers, donors, guests, etc.

    o Highly motivated and committed to Samaritan’s mission.

    o Bachelor's degree or equivalent experience.

    o Must be willing to work some early mornings and weekends.

    o Ability to travel to various event, sites and meeting locations as needed for the job.

    o Ability to lift up to 25 lbs.
