    Cayaba Care is building support services to reimagine how we manage acute illness and improve outcomes in complex populations. The US is one of the most dangerous developed countries to give birth and these poor maternal-fetal outcomes disproportionately impact minority women and underserved populations across major US cities. Our team is focused on closing this gap through a combination of clinical and non-clinical services delivered in-home or in-office. The Cayaba Care model focuses on integration not replacement of patients' primary Obstetric provider and reducing any barriers to the patient receiving the recommended care. * * What You'll Do

    • As a social worker, you will direct our patients to health and social services.

    • You will provide case management to pregnant women through home and office visits, as well as phone communication.

    • You will develop relationships with relevant communities, organizations and agencies.

    • You will ensure support for women in addressing issues relating to substance abuse, harm-reduction, and MAT options.

The OnCall Health care model focuses on integration not replacement of patients' primary Obstetric provider and reducing any barriers to the patient receiving the recommended care. * Who You Are

    • You are knowledgeable about community health issues relating to pregnant women and services for mothers and families.

    • You are able to travel locally to carry out home visits.

    • You are familiar with GSuite, GMail, and familiar with computers/software.

    • You maintain accurate case documentation and client data.

    • You are organized and efficient.

    • Preferred Credentials
    • Fluent in Spanish

    • Bachelor's Degree in Social Work

    • Experience with community health issues

    • Valid Driver's License
