Seeking a PUBLIC HEALTH NURSE for a 2 month contract with an option to extend
• Full time (35 hours) public health nurse to be co-located with Child Protective Services
• Accompany CPS and preventive caseworkers in the field to visit with identified families using a screening tool (e.g. Uncope+) and/or provide direct consultation with CPS/preventive caseworkers regarding families who might benefit from substance abuse screening
• Conduct screenings for adults and/or children from early identification of substance abuse treatment needs
• Assist in the development, implementation, and monitoring of the plan of safe care for any infant born to a parent with a positive toxicology screen, and infant identified as being affected by substance abuse or withdrawal symptoms or a Fetal Alcohol Spectrum Disorder
• Provide clinical assessments and documentation to support the CPS process
• Use knowledge of resources within the local district to provide referrals for identified children and/or adults who may need ongoing services
• Assist caseworkers in assessing the medical needs of children and families involved with CPS cases
• Facilitate medical services for children placed in foster care
• Complete the plan of safe care and continuous monitoring for services compliance on open CPS cases
• Conduct drug screenings for parents involved in CPS investigations
• Attend Family Team Meetings, Permanency Planning Reviews, Service Plan Reviews in reference to the medical needs of children placed in foster care/and or children involved in open CPS investigations
• Teaching in home medical procedures for children placed in foster care to caretakers
• Request medical records from hospitals and doctors office for children placed in foster care
• Enter progress notes/records into the statewide CPS database system
