    Position Summary:

    Provides counseling services including identifying, screening and assessing clients to determine the need for substance use treatment. Provides referrals to community-based organizations and coordinates care as needed. Responsible for facilitating therapeutic group and individual sessions. Responsible for completing and maintaining detailed OASAS-compliant case records.

    Direct Supervisor: Director of Behavioral Health Services

    Duties and Responsibilities:

    • Conduct intake interviews, biopsychosocial, mental health screenings.
    • Formulate treatment plans and treatment plan reviews, act as participant’s case manager to coordinate care within Crossroads and will external providers.
    • Perform individual counseling duties which include holding weekly individual sessions with clients, providing referrals to community-based resources (healthcare, social services, and administrative e.g. help obtaining insurance), and developing client schedules.
    • Initiate and maintain client records in accordance with policies and procedures.
    • Ensure accurate and timely data entry of all client information into the agency’s database; utilize electronic health record system.
    • Facilitate assigned treatment groups.
    • Perform drug testing as needed.
    • Prepare monthly updates for court or other supervising agencies.
    • Participate in treatment team case reviews.
    • Consult with other professionals in support of client’s treatment.
    • Complete discharge documentation as needed.

    Qualifications:

    • LMSW or CASAC is required (CASAC is also preferred for social workers)
    • Minimum of two years’ clinical/case management experience in working with clients that have substance use disorders is required.
    • Experience working with the criminal justice population is preferred.
    • Computer literacy (word processing, e-mail, internet research, data entry) is required.
    • Ability to use an electronic health record system required.
    • Bilingual (English/Spanish) preferred.
    • Relevant personal experience in terms of recovery and/or justice involvement, a plus.
