Anthem Hospice is looking for Social Workers who are passionate about their work.

JOB RESPONSIBILITIES:

Social Workers should be patient, calm and able to manage stress. They work directly with patients and their families and need to have compassion. They may need to provide treatment to patients in lieu of medical professionals in the event of emergencies, while also providing therapeutic support as needed. A successful Social Worker will have strong communication skills, both to provide understandable support and plans to patients and their families and to listen carefully to understand their issues. They must be patient when talking to patients and their families and know how to calm them down and solve their problems. It is important to have empathy so they can show the patient they understand their issue and are working to help them.

OTHER RESPONSIBILITIES:

Social Workers may also include the following duties:

· Build rapport with patients and their families

· Develop care plans for the patients

· Evaluate patients (existing, new and prospective) based on their needs, limitations and desires

· Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager

SKILLS AND QUALIFICATIONS:

· Bachelor’s or master’s degree in social work

· 5+ years of clinical social work experience

· Knowledge of specific software program used within your organization

· Valid and current MSW license

· Excellent listening and communication skills

· Ability to make psychological assessments and develop and implement care plans

· Ability to provide after-hours service as needed

· Hospice experience preferred

· Bilingual preferred

Job Type: Per Diem Position

Pay: $65-$70 per visit

Salary: $50,000.00 - $75,000.00 per year
