    Vitality Hospice & Palliative Care

    Vitality Hospice & Palliative Care is a clinician-owned and operated hospice seeking a compassionate and dependable Certified Home Health Aide (CHHA) to join our growing interdisciplinary team.

    In this field-based role, you will provide hands-on personal care and emotional support to patients in their homes, helping ensure comfort, dignity, and quality of life throughout their end-of-life journey. This is a meaningful opportunity for caregivers who value autonomy, purpose-driven work, and a supportive team environment.

    Schedule & Position Details

    • Full-Time, Monday–Friday
    • Typical hours: 7:00 AM – 3:00 PM
    • Occasional evening and weekend overtime available
    • Field-based position with local travel
    • ** Mileage reimbursement provided**
    • Hiring immediately

    Key Responsibilities

    Under the supervision of a Registered Nurse Case Manager, the CHHA will:

    • Assist patients with mobility, transfers, and ambulation
    • Provide personal care including bathing, grooming, dressing, and feeding
    • Offer companionship and emotional support to patients and families
    • Perform light housekeeping tasks related to patient care
    • Accurately document care provided in a timely manner
    • Follow individualized care plans and hospice standards
    • Promote a culture of compassion, respect, and patient-centered care

    Qualifications

    • Active Certified Home Health Aide (CHHA) certification (required)
    • Valid driver's license, reliable vehicle, and current auto insurance
    • Ability to work independently in the field
    • Strong communication, time-management, and organizational skills
    • Compassionate, dependable, and patient-focused
    • Hospice, home health, or palliative care experience preferred (not required)
