    Certified Nurse Aide (CNA) – Home Health & Hospice

    Full-Time / Part-Time Positions Available

    Job Summary:

    We are seeking compassionate and reliable Certified Nurse Aides (CNA) to join our Home Health and Hospice team. The CNA will provide high-quality, patient-centered care to clients in their homes or hospice setting under the supervision of a Registered Nurse (RN) or Licensed Vocational Nurse (LVN).

    The ideal candidate is dependable, caring, and committed to supporting patients with dignity and respect during their care journey.

    Key Responsibilities:

    Provide personal care services including bathing, grooming, dressing, and toileting

    Assist patients with mobility, transfers, and ambulation

    Monitor and report changes in patient condition to supervising nurse

    Assist with vital signs (temperature, pulse, respiration, blood pressure)

    Support patients with activities of daily living (ADLs)

    Maintain a clean, safe, and comfortable environment for patients

    Follow care plans as directed by the RN/LVN

    Document care provided accurately and timely

    Comply with all agency policies, HIPAA, and safety standards

    Education & Certification Requirements:

    Must be a Certified Nurse Aide (CNA) with a valid CNA license in the state of Texas (or applicable state)

    High School Diploma or GED preferred

    Current CPR certification (preferred or required depending on agency policy)

    Experience Requirements:

    Prior experience in Home Health, Hospice, Nursing Home, or Hospital setting preferred

    Experience working with elderly, disabled, or terminally ill patients strongly preferred

    Familiarity with ADLs and patient care documentation

    Skills & Competencies:

    Strong communication and interpersonal skills

    Compassionate and patient-centered attitude

    Ability to work independently in patient homes

    Reliable transportation and valid driver’s license (if required for field visits)

    Strong attention to detail and documentation accuracy
