Job Summary

We are seeking a Certified Nursing Assistant (CNA) to join our team for a health coach position to do monthly calls to Structured Family Care Clients and caregivers

Responsibilities

Monthly Phone Calls to members to answer care questions Form given for completion

Completing 6 Month in person Visits while on the phone with members case manager also form given for completion

Working with our Medicaid Nurse who oversees all Health coach visits and 6 month supervisory visit

Health Coach Team Meetings

Qualifications

High school diploma/GED

Valid Certified Nursing Assistant (CNA) certification

First aid and CPR-certified

Basic computer skills

Ability to adhere to all health and safety guidelines

Excellent communication and interpersonal spills

Ability to lift heaving objects

Valid driver’s license and reliable transportation

Ability to work with limited supervision

Compassionate, respectful, ethical
