Location: Santa Fe Springs and Downey, CA
Rate: $26-$27/hour

The Certified Nursing Assistant (CNA) plays a crucial role in providing direct care and support to residents at both Pathway Home (Santa Fe Springs) and the Hondo Center (Downey). This role requires an even distribution of work time between the two sites, with site assignments subject to change based on program needs.
This position involves assisting residents with daily living activities, supporting healthcare services, and promoting a safe and compassionate environment. The CNA will work closely with medical professionals, social workers, and other shelter staff to ensure the well-being of residents.
Key Responsibilities
Health Monitoring and Direct Medical Support:
- Measure and document residents' vital signs, including blood pressure, blood sugar levels, temperature, and pulse.
- Monitor residents for overdose risks, withdrawal symptoms, and related health concerns.
- Monitor resident compliance with prescribed medications as directed by healthcare professionals.
- Assist with basic healthcare tasks under the supervision of medical staff.
- Provide first aid and respond to medical emergencies in accordance with training and protocols.
- Collaborate with shelter team members to coordinate resident care and support services.
Medication Coordination and Management:
- Coordinate prescription assistance, medication pick-up, and delivery.
- Maintain accurate records of medication schedules and promptly report concerns to nursing or medical staff.
- Coordinate medical appointments, hospitalization, and care access
- Support residents in preparing for medical appointments, treatments, and transportation.
- Encourage resident participation in scheduled health appointments.
- Assist residents in accessing and understanding available healthcare services.
- Coordinate, schedule, and facilitate sign-ups for mobile clinics and vaccinations.
- Provide support for client hospitalizations, including admission reasons and discharge coordination.
Documentation and Medical Records Management:
- Accurately document care provided and resident observations in accordance with shelter standards.
- Document and report changes in residents' health or behavior to appropriate medical personnel.
- Maintain client medical face sheets, ensuring information is complete, current, and reviewed regularly.
- Maintain accurate, up-to-date client medical files in compliance with shelter and regulatory requirements.
Safety, Infection Control, and Living Environment:
- Ensure residents' living areas are clean, safe, and conducive to health and well-being.
- Follow infection control protocols to maintain a safe environment for residents and staff.
- Assist residents with hygiene supplies as needed.
Specific Skills Required/Desired
Required:
- Current Certified Nursing Assistant (CNA) certification in California.
- Ability to pass a background check and drug screening
- Good communication skills and the ability to work well in a team.
- A compassionate and caring attitude toward helping people in need.
- Basic computer skills for maintaining records and communication.
Desired:
- Experience in a healthcare or shelter setting.
