Job Details:

Locations: Jacksonville, FL (Clay, Duval, and St. Johns Counties)

Salary: $16.00 - $18.00 per hour

What You’ll Do:

Deliver personalized care services in clients’ homes

Assist with daily living activities and personal hygiene

Manage household tasks and provide transportation to appointments

Monitor and report clients’ health status using the WellSky App

Maintain detailed care records

Provide companionship and emotional support to enhance clients’ well-being

What You Need:

Current Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification

Experience in home health care (1+ year preferred)

Level 2 background screening

CPR certification

Current TB test (within the last year)

Valid driver’s license, proof if social security and reliable transportation
