Certified Nursing Assistant / Home Health Aide – Job Description

Job Summary

We are seeking a compassionate and reliable Certified Nursing Assistant (CNA) or Home Health Aide (HHA) to join our Home Care team. You will provide one‑on‑one care in patient homes, following individualized care plans designed to support safety, comfort, and independence. The ideal candidate is patient‑focused, detail‑oriented, and committed to creating a healthy, supportive environment for every client. Strong communication skills and a genuine passion for helping others are essential. Prior Home Health, Hospital, Nursing Center or Hospice experience is a plus.

Responsibilities

Assist patients with daily activities, including transfers, bathing, dressing, grooming, toileting, and mobility.

Support patients with household tasks such as changing bed linens, laundry, light cleaning, and meal preparation.

Observe and report changes in patient condition, behavior, or needs to the supervising nurse.

Maintain accurate records of care, progress, and concerns.

Provide companionship and emotional support, including reading aloud, conversation, and engagement in mentally stimulating activities.

Assist with range‑of‑motion exercises and other therapeutic activities as directed by the care plan.

Note: CNAs/HHAs may assist with medication reminders but do not administer medications.

Qualifications

Completion of an approved CNA or Home Health Aide training program.

Minimum one year of professional caregiving experience preferred.

Valid Driver’s License.

Current CPR certification.

Strong communication, professionalism, and respectful interpersonal skills.

Ability to work independently, follow care plans, and maintain patient confidentiality.
