    Position Summary

    The Certified Nursing Assistant (CNA) plays a key role in supporting the facility’s chronic care management (CCM) initiatives. This position works closely with nurses, providers, and interdisciplinary teams to ensure residents with chronic conditions receive consistent, proactive, and comprehensive care. The CNA contributes to improved resident outcomes by assisting with ongoing monitoring, documentation, and communication that help prevent avoidable hospitalizations and promote resident well-being.

    Key Responsibilities Resident Monitoring & Support

    Perform routine vitals, weight monitoring, and symptom checks as directed by the nursing team.

    Observe and report changes in residents’ condition promptly to licensed nursing staff.

    Support chronic disease management protocols (e.g., CHF, COPD, diabetes, hypertension) through scheduled assessments and check-ins.

    Documentation & Care Coordination

    Accurately document resident data in the electronic health record (EHR) and chronic care workflows.

    Assist nursing staff in gathering information needed for CCM reviews, care plan updates, and interdisciplinary team meetings.

    Ensure timely communication of resident needs, risk factors, and early warning signs.

    Resident Engagement

    Encourage residents to participate in their chronic care plans, including hydration, mobility, medication routines, and therapy activities.

    Provide education reinforcement based on nursing instructions (e.g., diet reminders, activity prompts, daily routines).

    Clinical Support

    Assist nurses and providers with chronic care procedures within CNA scope, such as positioning for pulmonary care, monitoring oxygen use, or collecting basic clinical data.

    Facilitate telehealth check-ins or remote provider assessments by preparing the resident and equipment.

    Operational Support

    Collaborate with nursing leadership to implement chronic care pathways and reduce hospital readmissions.

    Help track follow-up tasks, transitions of care, and scheduled chronic care touch points.

    Qualifications

    Active Certified Nursing Assistant (CNA) license in state of employment.

    Experience in a Skilled Nursing Facility or long-term care setting preferred.

    Strong understanding of chronic disease symptoms, early warning signs, and the importance of proactive care.

    Excellent communication, observation, and documentation skills.

    Ability to work collaboratively with nurses, providers, and interdisciplinary teams.

    Compassionate, patient-centered approach to resident care.

    Physical Requirements

    Ability to lift, move, or assist residents as needed.

    Ability to stand, walk, bend, and perform physical tasks throughout the shift.
