    Position Summary:

    The Chief Strategic Initiatives Officer (CSIO) is a key member of the YNHS Executive Leadership Team, responsible for driving strategic alignment between social services and clinical outcomes. This role leads the Social Services Department, oversees multiple local, state, and federal grants, and ensures that Health-Related Social Needs (HRSNs) are effectively addressed to improve patient health outcomes. The CSIO serves as the primary liaison between community-based services and clinical operations, ensuring integrated care delivery across the organization.

    Key Responsibilities:

    Strategic Leadership & Integration

    - Lead strategic initiatives that connect HRSNs to clinical outcomes, ensuring alignment with YNHS’s mission and goals.

    - Collaborate with the Chief Medical Officer, Chief Quality and Compliance Officer, and other C-suite leaders to integrate social services into clinical workflows.

    - Represent YNHS in community partnerships, coalitions, and policy forums related to social determinants of health.

    Program Oversight

    - Directly supervise staff overseeing housing, youth, emergency services, and self-sufficiency programs.

    - Provide and oversee leadership to 25–30 direct services staff including case managers, outreach workers, and supportive housing personnel.

    - Ensure culturally competent, trauma-informed service delivery across all social service programs.

    Grant Management

    - Manage a portfolio of local, state, and federal grants including budgeting, reporting, compliance, and renewal.

    - Collaborate with the Development Director and Finance Department to ensure fiscal accountability and strategic use of funds.

    - Monitor program performance metrics and outcomes to meet grant requirements and improve service delivery.

    Operational Excellence

    - Develop and implement policies and procedures that support effective program operations and staff development.

    - Use data to drive decision-making, evaluate program impact, and inform strategic planning.

    - Ensure compliance with all regulatory and funding requirements.

    Qualifications:

    - Master’s degree in Public Health, Social Work, Public Administration, or related field (or equivalent experience).

    - Minimum of 7 years of progressive leadership experience in social services, healthcare, or nonprofit management.

    - Proven experience managing complex grants and budgets.

    - Strong understanding of HRSNs and their impact on clinical outcomes.

    - Exceptional communication, collaboration, and leadership skills.

    - Experience working in a Federally Qualified Health Center (FQHC) or similar setting preferred.
