    PRIMARY FUNCTION/PURPOSE:

    Develop, implement, and supervise the delivery of services provided by Client Care Coordinator Supervisors/Social Work Supervisors, Client Care Coordinators/Social Workers and CASACs to enhance existing services to homeless individuals/ families to include the completion of a thorough assessment and the development and completion of bio-psychosocial/CASAC assessment of each individual/family upon intake, crisis intervention, counseling and client advocacy and referrals to government and community-based resources. Develop and institute a program where achievable goals will be implemented on each individuals/family's service plan leading to permanent housing and strengthen the overall permanency outcome for individuals/families with children in shelter. Work in collaboration with the Senior Vice President of Programs & Staff Development to provide clinical development of the social work and CASAC team.

    PRIMARY RESPONSIBILITIES:

    • Oversee and supervise the development of the Client Care Coordinator/Social Work and CASAC Team.
    • Oversee administrative, operational, programmatic issues that might affect client care coordinator/social worker and CASAC staff.
    • Work collaboratively with the Program Directors in developing the process of distribution of cases to client care coordinators/social workers & CASACs for newly admitted individuals/families to be assessed immediately upon admission and managed throughout shelter stay.
    • Ensure the timely completion and quality control of comprehensive bio psychosocial and CASAC assessments for each individual/family to understand strengths and service needs.
    • Work effectively as a part of a multi-disciplinary team to enhance client engagement and the delivery of quality service.
    • Enhance the delivery and coordination of mental health and substance abuse and related services to individuals and families with children in shelter.
    • Ensure that Client Care Coordinators/social workers and CASACs reports are clearly written and capture individual/family assessment findings and recommendations.
    • Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and wellbeing.
    • Ensure Client Care Coordinator Supervisors/Social Work & CASAC Supervisors are providing clinical oversite to Client Care Coordinators/social workers and CASAC’s in providing short-term counseling, outreach services, crisis intervention, risk assessment, safety planning and pyscho-education for individuals and families.
    • Collaborate with ACS and /or prevention services agencies and participate in conferences to advocate on behalf of family as needed.
    • Ensure that Client Care Coordinators/social workers and CASAC’s are providing individuals and families with appropriate referrals and linkages between mental health and substance abuse providers, government agencies and other related community based services as needed.
    • Serve as agency/program liaison between DHS, government agencies, community-based organizations and/or groups.
    • Develop and implement group work for individuals and families to be facilitated by client care coordinators/social workers and CASACs to address individual and family issues, enhance life skills and etc.
    • Ensure that Client Care Coordinators/social workers and CASACs are completing all documentation in a timely manner.
    • Ensure that Client Care Coordinators/social workers assess each individual/family for supportive housing, complete supportive housing applications when applicable, and accompany residents to supportive housing interviews when needed.
    • Ensure that Client Care Coordinators/social workers complete and discuss exit summaries with each individual/family and provide individuals/families with referrals and supportive services identified in new community.
    • Participate in individual/family meetings with case work and housing team to discuss planning efforts and progress as needed.
    • Network and establish successful linkages with government and community based organizations to create resources for both individuals, families and staff.
    • Work in collaboration with the Director of Social Services to ensure that all services lead to permanent housing placement.
    • Complete special projects and assignments as needed or identified by the Executive Director.
    • Provide on-call crisis intervention support to client care coordinators and CASACs.
    • Perform other duties as assigned.
    • Develops training curriculums and provides training to all program services staff.
    • Provide site clinical case consultation services regarding challenging clients or programmatic issues that might affect the delivery of clinical services by the social work and CASAC team.

    POSITION REQUIREMENTS AND MINIMUM QUALIFICATIONS:

    Job-related knowledge:

    • Experience with multi-problem singles/families, management, professional training & development, supervision, and social services.

    Job-related skills, including language, mathematical, and reasoning (analytical) skills:

    • Knowledge of office systems; MS-Office preferred. Process Improvement, Analyzing Information, Strategic Planning, Verbal Communication, Emphasizing Excellence, Attention to Detail, and dealing with multi-site organizational issues.

    Intellectual, creative, and/or communication abilities:

    • Excellent written and oral communication, and organizational skills.
    • Must have incredible initiative, the ability to work well under pressure, and the ability to work both independently and as part of a team to complete complex projects in a fast-paced environment.
    • Must be adept and comfortable with crisis management.

    Licenses, certifications, and degrees:

    • To perform this job successfully, and individual must be able to perform each essential duty satisfactorily. Master’s Degree in social service field and at least eight (8) years of related work experience in a non-profit executive director position at a licensed agency.
    • LCSW or LCSW-R license required.

    Other Requirements:

    • May be required to work long hours and a varied schedule.
    • Travelling throughout various locations in the New York City area.
    • Lifting and moving objects weighing up to 30 pounds several times a week.
