Job Description
POSITION SUMMARY
Screens and identifies older adults for Alzheimer’s disease and related dementias (ADRD). Delivers Savvy Caregiver Program to family caregivers of older adults, and offers family-based therapy, individual counseling and consultation. Conducts ADRD awareness campaign and contributes to the development of the ADRD website.
DUTIES AND RESPONSIBILITIES
Delivers Savvy Caregiver Program to family caregivers of the older adults with Alzheimer’s disease and related dementias (ADRD)
Offers family-based therapy and individual counseling to families of older adults with ADRD
Offers consultation to the families of older adults with ADRD
Screens and identifies older adults for ADRD
Conducts ADRD media campaign
Contributes to the development of a ADRD website
Contributes to the annual ADRD awareness campaigns
Prepares regular reports
Facilitates inter-disciplinary care team meetings
Develops and maintains relationships with the community partners on the Alzheimer’s Disease Programs Initiative Project
Participates in and contributes to Project Team Meetings, Training Sessions, and Department Meetings
Be responsible for own professional development, including participating in the supervisory process and using other educational opportunities, both within and outside of the agency
Be responsible for data management using Salesforce.

EDUCATION/EXPERIENCE:
· Master degree in Social Work and obtain a License in Social Work within six months upon hire.
· License in Social Work or License in Clinical Social Work preferred.
· Previous experience in social work, geriatric, counseling or related fields preferred.
SKILLS/KNOWLEDGE:
Ÿ Spoken and written knowledge of Chinese and English. (Cantonese preferred, Toishanese and Mandarin a plus)
Ÿ Compassionate and caring demeanor
Ÿ Ability to build rapport with clients
Ÿ Access to personal transportation
Ÿ Access to variety of diagnostic tools and therapy materials
Ÿ Basic end-user level understanding and experience with Salesforce is a plus

Programs Child Education & Development Services helps children learn in an enriching bilingual/bicultural environment,and equips them with the skills to succeed at school. We support families in their efforts to provide their children with a firm foundation and Chinese cultural retention as they transition into American culture. CEDS - consists of three programs for families with children, spanning the age range from birth to 12 years old. The Elderly Services Department is dedicated to enabling older adults to achieve healthy living and maintain independence through our person centered, culturally competent, and integrated home and communitybased services. The Employment and Training Department provides a variety of programs that assist job seekers—particularly new immigrants—in the greater Chicago area to search for employment. We also assist youth participants in obtaining their high school diploma and pursue their career by attending postsecondary education. In addition, we provide transitional job training to seniors who are 55 years and older, and assist them with unsubsidized placement. Family and Community Services (FCS) aims to assist and strengthen the physical and mental wellbeing of individuals and families of all ages and backgrounds in the Chinese community. FCS helps clients’ transition into American society, while maintaining their Chinese heritage and culture. The department also helps families and individuals access resources and services as related to health care, public benefits, school placement and adjustment. The Housing and Financial Education Department serves to educate and counsel low income and moderate income families on financial literacy and housing related issues. We meet the needs of the expanding community—especially the Chinese American community—by bringing in mainstream resources and assisting prospective homebuyers in realizing the American Dream.
