    Job Description: As a Home Health Registered Nurse Case Manager, you will play a pivotal role in coordinating and providing comprehensive healthcare services to our patients in their homes. You will work closely with interdisciplinary teams to ensure the highest level of patient care and achieve optimal outcomes. Your responsibilities will include:

    Conducting thorough patient assessments to develop personalized care plans

    Administering skilled nursing care in accordance with physician's orders and established protocols

    Monitoring and documenting patient progress, ensuring accurate and timely charting

    Educating patients and their families on disease management, treatment plans, and self-care techniques

    Collaborating with physicians, therapists, and other healthcare professionals to coordinate and ensure seamless care transitions

    Participating in case conferences and interdisciplinary team meetings to review patient progress and adjust care plans as needed

    Complying with all regulatory guidelines, including OASIS assessment and documentation

    Qualifications:

    Active Registered Nurse (RN) license in the state of Florida

    Minimum of 2 years of experience in home health care (preferred)

    Proficiency in OASIS documentation, required

    Ability to exercise initiative and independent judgment

    Strong clinical and assessment skills

    Excellent communication and interpersonal skills

    Proficient computer skills for accurate documentation and electronic medical records

    Reliable transportation and valid driver's license
