The Chronic Care Management (CCM) Coach plays a critical role in Carda Health's programs by performing chronic care management services under the supervision of the nurse practitioner for patients who are participating in Phase 3 Pulmonary Rehab. This role encourages patients to reach their health goals while maintaining patient safety, evaluates medical appropriateness for exercise-based programs, is responsive to patient's emergent medical needs, and supports the Carda Health team in maintaining a high standard of care aligned with clinical guidelines. Candidates should have experience in patient care in a clinical setting, be comfortable in emergencies situations, be comfortable with live video telehealth visits, have a thorough knowledge of common disease processes such as COPD, CAD, T2DM, be aware of therapies and medications involved with those diagnosis, feel comfortable with various tech platforms and be adaptable for fast paced changes in the clinical setting.
