Chronic Care Manager / Supervisor

Desert Ridge Family Physicians is looking for a Chronic Care Manager / Supervisor to lead our Chronic Care Management (CCM) team. In this dual-role, you’ll provide hands-on care to patients with chronic conditions and guide a team to deliver exceptional, coordinated care.

Key Responsibilities
Chronic Care Management
Collaborate with physicians, patients, families, and healthcare staff to coordinate chronic care programs.
Develop and implement individualized care plans aligned with patients’ medical needs and personal goals.
Educate patients on preventive care, medications, and community resources.
Conduct medication reconciliation and follow-up assessments to monitor ongoing progress.
Reduce ER utilization and hospital readmissions through proactive care coordination.
CCM Supervisor Responsibilities
Monitor employee performance and provide feedback and coaching to build skills and confidence.
Train and onboard new employees to ensure consistency and quality of care.
Manage staff schedules, approve time off requests, and oversee workflow and productivity.
Implement process improvements and ensure regulatory compliance.
Serve as the bridge between leadership and front-line staff, reporting team metrics and outcomes.
Facilitate team meetings to ensure alignment and collaboration.
Who You Are
Experienced Chronic Care Manager or care coordinator.
Minimum 3 years as a Medical Assistant (certification preferred).
Proven leadership or supervisory experience required.
Strong organizational, problem-solving, and communication skills.
Compassionate, patient-centered, and eager to improve care outcomes.
Experience with Athena EHR or similar systems is preferred.
