Job Description:

Transport residents to appointments and perform direct resident care duties under the supervision of licensed nursing personnel, as needed. Assist with promoting a compassionate physical and psychosocial environment for the residents.

Responsibilities:

• o Requires heavy physical work; heavy lifting, pushing, or pulling required of objects up to fifty (50) or more pounds. Physical work is a primary part (more than 70%) of job.
• o Push, pull, move, and/or lift a minimum of fifty (50) pounds to a minimum height of three (3) feet and be able to push, pull, move, and/or carry such weight a minimum distance of fifty (50) feet.
• o Standing and/or walking for more than four (4) hours per day. o Bending and/or stooping for more than one (1) hour at a time.
• o Sitting for more than two (2) hours at a time.
• o Repetitive gripping for more than two (2) hours per day

Requirements/Qualifications:

• Identify and participate in process improvement initiatives that improve the customer experience, enhance work flow, and/or improve the work environment.

• • Confirm doctors’ appointments for residents. • Assist in making arrangements, and in providing actual transportation, to appointments, community events and others, as needed. • Notify family of arrangements in regards to appointments.

• • Monitor daily documentation and filing. • Perform clerical tasks to include processing administrative transfer and discharges. • Provide personal care (i.e., grooming, bathing, dressing, oral care, etc.) of residents daily and as needed

• Meet physical and sensory requirements stated below, and be able to work in the described environment.

• • Utilize proper body mechanics when lifting or transporting residents. • Assist in the orientation and training of other staff, as requested. • Other special projects and duties, as assigned.
