    Overview: Outreach Case Managers will be part of the Community Care Response Team responding to 9-1-1 and Community calls for service for people experiencing homelessness and mental health concerns. Case Managers will be part of multi-disciplinary care teams alongside Social Workers, Drug and Alcohol Counselors, Psych Techs, and other professionals offering care and service coordination in partnership with other nonprofits, County and City entities, law enforcement, behavioral health services, and medical providers to ensure clients receive the help they need. These services are provided in collaboration with our partners with the intention of assisting clients with obtaining and maintaining permanent housing as quickly as possible.This position works four days per week, 10 hours each shift (4/10s Alternative Workweek Schedule)

    Shift 1: 10:30 am - 9:00 pm (Sunday - Wednesday)

    Shift 2: 7:00 am - 5:30 pm (Wednesday - Saturday)

    Case Managers (CMs) provide case management and supportive services to individuals and families who are experiencing street-level homelessness. Through street outreach, CMs seek to connect unsheltered homeless neighbors to sustainable housing. CMs meet clients where they are and support them to achieve their goals. City Net’s services are housing-focused; CMs work with clients on developing long-term housing plans. CMs connect homeless neighbors to appropriate supportive services, including medical health treatment, mental health treatment, counseling, and permanent housing.

    Case Managers offer care and service coordination in partnership with other nonprofits, County and City entities, law enforcement, behavioral health services, and medical providers to ensure clients receive the help they need. These services are provided in collaboration with our partners with the intention of assisting clients with obtaining and maintaining permanent housing as quickly as possible. City Net’s programs follow Housing First and Harm Reduction service models.Requirements

    Ability to work well with people from diverse backgrounds with varying degrees of experience

    This position may involve variable scheduling including early mornings, late evenings, and weekends. For example, there may be an early morning shift once per week, or a weekly evening outreach. Must be available for this type of variable schedule.

    May be required to drive a company vehicle to transport clients. For insurance purposes, drivers must be at least 25 years of age, have a valid CA Drivers License, and must pass a Motor Vehicle Report.

    Must be authorized to work in the United States for any employer.

    Constantly works in outdoor weather conditions.

    Frequently moves about inside the office, in the field (both indoors and outdoors at local shelters, parks, restaurants, riverbeds, beside railroads, beside freeways, etc.), or at meetings/events to access people, file cabinets, office machinery, etc.

    Frequently ascends/descends stairs, hills, and underpasses to access clients.

    Frequently position self to communicate effectively with clients sitting or lying on the ground.

    Must be able to traverse rough outdoor terrain at least 2/3 of the time.

    Must be able to move oneself quickly in certain urgent/emergent situations (e.g. to avoid falling or projected items, or to administer Naloxone/Narcan to a client who has overdosed).

    Proof of vaccination or weekly testing required. All City Net employees working in California are required by the California State Public Health Officer Order of July 26, 2021 to either be fully vaccinated or to submit proof of weekly testing for COVID-19.

    Nice To Have

    Case management experience with City Net’s target populations (people affected by homelessness, substance use disorders, chronic health and mental health concerns, domestic and intimate partner violence, and/or human trafficking) strongly preferred.

    Experience and homelessness services professional contacts in Orange County and surrounding areas.

    Experience gathering and maintaining confidential client records, including intake paperwork, case notes, and client management system data entry.

    Bachelor’s degree or higher in Social Work, Human Services, Sociology, Psychology, or a related field
