AmeriCorps/Interns service with Neighbor Support Services with the Destination Home housing program. Primary duties will focus on the coordination of placing neighbors who are experiencing homelessness and who live with physical disabilities, mental illness, and/or substance abuse disorders into Destination Home permanent housing and other potential NSS housing programs. AmeriCorps will also provide supportive services to those placed in Destination Home. * Interview and assess individuals to determine eligibility for program, including conducting assessments. * Provide neighbors with information and referrals and linking the neighbors with treatment for substance abuse and mental health services, as well as other resources that are needed (food, clothing, etc) both within CitySquare and outside of CitySquare. * Assisting neighbors in obtaining critical documents needed for housing. * Verify all necessary documentation and proof of neighbor eligibility. * Assist neighbors with move-in process. * Work closely with case managers, outreach workers, and emergency shelters to assure housing placements are made efficiently and appropriately. * Assist housing staff with intake procedures. * Facilitate housing orientations for new participants which include explaining rules, policies, program expectations and terms of their lease. * Assist in housing inspections for neighbors. * Assist and arrange for neighbors to be moved to new units as needed. * Conduct neighbor follow-ups. * Facilitate/plan supportive services, life skills and/or other group activities as needed. * Maintain current documentation and records of encounters, assessments and referrals, as well as other statistical data required by the program in ETO and HMIS. * Attend staff and team meetings.
