    As a healthcare organization dedicated to providing quality services to the Central Coast community, our client is seeking a highly motivated individual to join their team. The ideal candidate will be committed to the mission of improving the health and well-being of members of the community and will bring a combination of skills, experience, and passion to the role.

    The Behavioral Health Community Integration Specialist supports individuals with mental health and substance use challenges. They report to the Behavioral Health Operations Manager and assist in coordinating care, including reviewing medical records, offering case management support, and facilitating transitions. Their role involves building relationships with the community, mental health plans, and providers. They provide information, referrals, and screenings, collaborating with various teams and entities. This position also maintains connections with county mental health plans and helps integrate the Behavioral Health Department into the community.

    Duties and Responsibilities

    • Assist members in accessing mental health care through coordination and education.
    • Coordinate care for regional center members in need of Behavioral Health Team services.
    • Fulfill responsibilities as a liaison with regional centers as outlined in the Memorandum of Understanding.
    • Provide crisis support to members and Behavioral Health staff.
    • Act as a clinical consultant to Mental Health Providers for case consultations and referrals.
    • Develop training programs for departments, providers, and communities in collaboration with Department Leadership and Provider Services.
    • Participate in interdisciplinary care team meetings.
    • Follow health plan policies, HIPAA, privacy laws, and regulatory standards.
    • Educate members, providers, and vendors about healthcare delivery and benefits related to behavioral health and substance use services.
    • Empower members with community resources, educational materials, and self-management tools.
    • Establish relationships with County Mental Health contacts and collaborate with agencies and organizations.
    • Review and implement Managed Care Plan Responsibilities outlined in County MOUs.
    • Maintain accurate data and departmental records.
    • Attend community meetings to support member access to care.
    • Communicate effectively with healthcare providers and stakeholders.
    • Utilize Solution-Oriented approaches for quality monitoring and innovative care strategies.
    • Prepare for audits and represent the department during audits.
    • Stay updated on clinical knowledge related to mental health and developmental disabilities.
    • Provide timely reports to supervisors.
    • Perform other assigned duties.

    Qualifications

    • Professional demeanor.
    • Knowledge of DSM-V diagnosis and evidence-based treatment approaches.
    • Familiarity with County-level Mental Health services and community resources.
    • Strong multitasking, organizational, and time-management skills.
    • Ability to work independently and collaboratively in a team.
    • Effective communication skills by phone, in writing, and in-person.
    • Proficiency with computers and relevant software.
    • Problem-solving skills when dealing with provider issues.

    Education and Experience

    • Master's level license or registered AMFT, ASW, APCC.
    • 2 years of case management and care coordination experience, including suicide assessment and screening.
    • 1-2 years post-graduate experience with Medi-Cal population and/or County Department of Behavioral Health.

    Desired

    • Bilingual in Spanish.
    • 4 years post-graduate experience in substance use, social work, or the behavioral health field.
    • Preferred licensure: LCSW, LMFT, or LPCC.
    • Prior experience in a managed care setting.
